If your child is interested in playing lacrosse, please fill out the following form. Several free clinics are available. The goal is to provide an opportunity for players and families to meet the coaches, players, and learn about lacrosse. It will also give players an introduction to lacrosse in a fun and safe environment. Lacrosse sticks will be available for use. Parent volunteers are welcome (this is a free clinic it doesn't work without us parents).

Open Clinics for boys and girls entering grades 2nd through 6th will be held at Southwood Park 6150 S 725 E, Murray, UT 84107 days and times are listed on question 1, please list a suggested day/ time if these days don't work. We are planning to form a Murray and/or Cottonwood boys and girls team for birth years 2013-2017 Please attend any one of the available days. Please wear proper athletic attire. We can't wait to see you at the field!
